medical plan, use this App to find a doctor or hospital in the SmartCare network on the fly. Or
search for Tier 1 (lower cost) pharmacies near you to get the most benefit coverage for your prescription.

The PPO I, PPO II and PPO III plans with the SmartCare network are available in the following
areas:

Chicago, IL
Dallas/Ft. Worth, TX
San Antonio, TX
Houston, TX
Beaumont, TX

This innovative App – SmartCare Doctor, Hospital and Pharmacy Locator – will help Home Depot associates quickly and easily locate any doctor or facility, including urgent care facilities, in the SmartCare Network from their IPhone or Apple device. The App will leverage your current location to help you find a doctor or facility close by. Your search will include the provider’s name, address and phone number and a map showing the location and the distance to the location. This App also helps you find the Tier 1 pharmacies that are closest to you.

The App is free and easy to use. We will soon modify the App to remember the zip code previously entered, thanks to the reviewer suggestion below.
